How To Get a Medical Marijuana Card In Georgia
Step 1
Register With Wellife Center
Book and online appointment with a Wellife Center doctor as a patient or you as a caregiver. The booking fee for the appointment is $149. The doctor will ask you why you would like an MMJ card and the doctor will ask you health and medical history questions. The doctor will provide you with advice and guidance on whether medical marijuana is a good choice for you.
Step 2
Apply For A Medical Card
If approved by our doctor, your information will be passed to the Georgia Low THC OIL Registry. You (or you as a care giver) will need to sign a waiver along side our doctor as part of the process.
Georgia Department of Public Health will reach out to you to request your certificate and signed and notarized waiver.
Step 3
Receive Your Medical Card
A Low THC Oil Card will be issued to you and/or caregiver. You will be notified when the card is ready for pickup (within 15 business days) from a Georgia Public Health office that is convenient to you. The cards cost $25 and are valid for two years. After that time you will need to again consult with us to confirm continued eligibility and request that our doctor update and confirm the accuracy of your personal information in the registry.
